Request for Assistance 2025–2026 School Year

 

Our Request for Assistance form is available for anyone who has a concern about a student—including staff, families, and students themselves.

This process helps ensure every student receives timely and appropriate support.

 

How the Process Works

  1. A Request for Assistance form is submitted.
  1. If the concern involves a need for counseling, the grade-level counselor meets with the student.
  1. Our data analyst reviews key student information such as grades, attendance, and discipline.
  1. The Student Support Team (SST) discusses the request and determines the most appropriate support or intervention.
  1. We follow up with the referring party and provide as much information as can be appropriately shared regarding next steps.

Important Reminder

When completing the form, please provide as much detail as possible. The more context we have, the better we can understand the concern and support the student effectively.

We also encourage staff to post the link in their Google Classroom so students can submit requests directly if needed.
